MK-677 (Ibutamoren) is a non-peptide agonist of the ghrelin receptor and a potent stimulator of growth hormone (GH) secretion. Its use to support or treat debility (general weakness or frailty, often associated with aging or chronic illness) is supported by a growing body of scientific evidence, primarily from clinical trials involving older adults and patients with catabolic states. Several studies have shown that MK-677 increases circulating GH and insulin-like growth factor 1 (IGF-1) levels, which are important for maintaining muscle mass and strength.

In elderly subjects, daily administration of MK-677 has been associated with increased lean body mass, improved nitrogen balance, and in some studies, modest improvements in muscle strength and function. For example, a landmark study by Chapman et al. (1996, NEJM) demonstrated that MK-677 reversed diet-induced protein loss and increased fat-free mass in healthy elderly volunteers. Other trials have explored its potential to counteract muscle wasting in conditions such as hip fracture recovery and chronic illness.

However, while MK-677 reliably increases GH/IGF-1 and lean mass, the evidence for functional improvements (e.g., strength, mobility, reduction in falls) is less consistent or robust, and longer-term safety data are limited. Side effects such as increased appetite, edema, and potential insulin resistance also warrant caution. Overall, the scientific evidence supports its biological plausibility and some clinical benefit for debility, but further studies are needed to confirm efficacy and safety for this indication.
