Bifidobacterium species have been studied as probiotic interventions for Crohn’s Disease (CD), an inflammatory bowel disease characterized by chronic gastrointestinal inflammation. Several small clinical trials and animal studies have evaluated the effects of Bifidobacterium-containing probiotics on intestinal inflammation and clinical symptoms in CD patients. Some studies have shown modest benefits, such as reduced inflammatory markers and improved gut microbiota balance when Bifidobacterium is administered, often in combination with other probiotic strains (e.g., in the VSL#3 formulation). However, results are inconsistent, with some randomized controlled trials failing to demonstrate significant clinical remission or prevention of relapse compared to placebo. Systematic reviews and meta-analyses generally conclude that while probiotics, including Bifidobacterium, are safe for most CD patients, the current evidence does not strongly support their routine use for induction or maintenance of remission in Crohn's Disease. The mechanisms proposed for Bifidobacterium’s potential efficacy include enhancement of epithelial barrier function, modulation of immune responses, and competitive inhibition of pathogenic bacteria. Despite this, major gastroenterological guidelines do not recommend Bifidobacterium-based probiotics as a standard therapy for Crohn’s Disease due to insufficient high-quality evidence. In summary, there is limited scientific justification for their use, reflected in an evidence rating of 2 out of 5.
