Ligusticum species, such as Ligusticum chuanxiong (commonly known as Chuanxiong or Szechuan lovage), have a long history of use in traditional Chinese medicine (TCM) for treating menstrual cramps (dysmenorrhea) and related gynecological disorders. In TCM, Ligusticum is often included in formulations aimed at "invigorating blood circulation" and "dispersing blood stasis," concepts believed to address menstrual pain. The most famous traditional formula containing Ligusticum chuanxiong is "Dang Gui Shao Yao San," which is widely used for menstrual discomfort. Historical texts, including the "Shennong Bencao Jing," document its use for regulating menstruation and alleviating pain.

Despite its traditional use, scientific validation is limited. Some preclinical studies suggest anti-inflammatory and antispasmodic effects of Ligusticum extracts, which could theoretically help reduce uterine muscle contractions and pain. However, high-quality clinical trials in humans are scarce, and evidence for efficacy in treating menstrual cramps specifically is not robust. Therefore, the justification for using Ligusticum for menstrual cramps is primarily traditional, supported by centuries of use and anecdotal reports, with only preliminary pharmacological support and insufficient clinical trial data.
