Clary sage (Salvia sclarea) has a long history of traditional use for easing menstrual discomfort and cramps. In folk medicine, particularly within European herbal traditions, clary sage was often recommended for women experiencing painful periods or irregular menstrual cycles. The essential oil of clary sage has been used in aromatherapy and massage, purportedly for its antispasmodic and relaxing properties. Some studies suggest that inhalation or topical application of clary sage oil may reduce perceived menstrual pain, but the quality and size of these studies are limited.

On the scientific front, a handful of small pilot studies and trials have explored clary sage’s effects on menstrual pain, often in combination with other oils. For example, a 2012 randomized controlled trial found that abdominal massage with a blend containing clary sage oil reduced period pain more than placebo, but the effect cannot be attributed to clary sage alone. The proposed mechanisms include mild muscle relaxation and modulation of perceived pain through olfactory pathways, but these are not well established. Overall, the evidence base is small and not robust enough for strong scientific validation, but traditional use is consistent and widespread, supporting its use as a complementary approach.
