Tetrahydropalmatine (THP) is an alkaloid derived primarily from the Chinese herb Corydalis yanhusuo. Its use to alleviate cramps and muscle spasms is rooted in traditional Chinese medicine (TCM), where Corydalis has long been employed to relieve pain, including dysmenorrhea (menstrual cramps), abdominal pain, and muscle tension. The traditional rationale is based on its reputed ability to "move qi" and "invigorate blood," thereby reducing stagnation and associated spasmodic pain. Scientifically, there is limited direct evidence evaluating THP specifically for muscle cramps or spasms in humans. Some preclinical studies suggest that THP has analgesic and muscle relaxant properties, possibly due to its action on dopaminergic and other neurotransmitter systems in the central nervous system. However, these studies are mostly animal-based or in vitro, and do not specifically address muscle spasms or cramps as a primary outcome. Therefore, while traditional use supports THP for cramps and spasms, scientific validation is limited and indirect. The current evidence base does not robustly confirm its efficacy for this indication, and clinical trials in humans are lacking. Most of the support for this use remains traditional, with moderate but not strong historical backing and weak scientific corroboration.
