Cubeb (Piper cubeba), also known as tailed pepper, has a long history of use in traditional medicine systems, particularly in Asia and the Middle East. Historically, cubeb has been employed for respiratory conditions, including coughs, bronchitis, and asthma. This use is documented in traditional Unani, Ayurvedic, and some early European herbal texts. The main rationale for its use against cough (especially spastic or persistent cough) is due to its reputed expectorant and anti-inflammatory properties, as well as its aromatic oils, which were believed to soothe the respiratory tract. However, the scientific evidence supporting its efficacy for treating cough is extremely limited. Only a few in vitro or animal studies have evaluated the antimicrobial and anti-inflammatory properties of cubeb extracts, and these do not directly establish an antitussive effect in humans. No high-quality clinical trials have been conducted to confirm its effectiveness or safety for cough relief. Therefore, the use of cubeb for spastic cough is based primarily on traditional practices rather than validated scientific research.
