Calamus rhizome (Acorus calamus) has a long history of use in traditional systems of medicine, such as Ayurveda and Traditional Chinese Medicine (TCM), for respiratory ailments including cough, bronchitis, and throat irritation. In these traditions, calamus is thought to possess antispasmodic and expectorant properties, which are believed to help soothe coughs, especially those described as spastic or convulsive. The rhizome is sometimes prepared as a decoction or used in herbal mixtures targeting symptoms like persistent cough or throat spasms.

From a scientific perspective, there is limited modern clinical evidence directly supporting the use of calamus rhizome for treating spastic cough. Some in vitro and animal studies suggest that extracts of Acorus calamus may have antispasmodic and anti-inflammatory effects, which could theoretically justify its traditional use. However, these findings have not been robustly confirmed in human clinical trials for cough or related respiratory symptoms.

It is important to note that calamus contains β-asarone, a compound with known toxic and potentially carcinogenic effects, leading to regulatory restrictions in some countries. For these reasons, while traditional use is well-documented, scientific validation remains insufficient, and caution is advised regarding its safety and efficacy.
