The use of plants from the Ilex genus (such as Ilex aquifolium, Ilex paraguariensis, and Ilex pubescens) for the treatment of cough, including dry cough, is primarily rooted in traditional medicine. In Chinese traditional medicine, certain Ilex species (notably Ilex pubescens and Ilex asprella) have been used as part of multi-herbal remedies for respiratory issues. Similarly, in some South American cultures, infusions of Ilex paraguariensis (Yerba Mate) have been consumed to alleviate minor respiratory complaints, although not specifically dry cough. However, scientific evidence to support the efficacy of Ilex spp. for dry cough is lacking. There are no well-controlled clinical trials or robust pharmacological studies that specifically demonstrate antitussive effects for any Ilex species. Most available evidence is anecdotal or based on ethnobotanical records. Additionally, the active compounds in Ilex (such as saponins, flavonoids, and caffeine) do not have a clearly established mechanism for suppressing cough. Thus, while traditional use exists, particularly in East Asian and South American herbal practices, the quality and quantity of evidence are very limited, and there is no scientific consensus on its efficacy for dry cough.
