Asafetida (Ferula assa-foetida) has a longstanding history in traditional medicine, particularly within Ayurveda and Unani systems, for the treatment of respiratory conditions including coughs, often described as "damp" coughs (productive or phlegmy coughs). In these traditions, asafetida is considered to have expectorant and antispasmodic properties and is sometimes used in formulations aimed at clearing phlegm and relieving cough. The aromatic resin is thought to help in thinning mucus, aiding its expulsion, and reducing airway irritation.

However, the scientific evidence supporting these uses is limited. There are a few in vitro and animal studies suggesting that asafetida has some anti-inflammatory, bronchodilatory, and antimicrobial effects, which could theoretically support its use in respiratory conditions. For example, a 2011 study in the Journal of Ethnopharmacology indicated potential antitussive effects in mice, but there are no well-conducted human clinical trials that confirm efficacy or safety for cough, especially damp or productive cough. Thus, while traditional use is well documented and there is some preliminary scientific interest, robust evidence in humans is lacking, justifying a moderate evidence rating.
