Adrenal extract, derived from the adrenal glands of animals (usually bovine or porcine), has a long history in traditional and alternative medicine, especially in the early 20th century. It was widely promoted as a remedy for fatigue, "adrenal exhaustion," and to support the body's response to stress, believed to be linked to cortisol production. The rationale was that consuming glandular tissue might help support the corresponding human organ system. However, the scientific evidence supporting adrenal extract for cortisol or adrenal function is minimal. There are no robust clinical trials demonstrating its efficacy in supporting adrenal health or modulating cortisol levels in humans. Modern endocrinology favors direct hormone replacement (such as hydrocortisone) in cases of adrenal insufficiency, rather than glandular extracts. Most professional organizations and regulatory agencies do not endorse adrenal extracts for medical use, and their safety and standardization are questionable. While adrenal extracts are still found in some dietary supplements, their use is based primarily on tradition and anecdotal reports rather than solid scientific validation.
