The adrenal cortex is the outer portion of the adrenal gland and is responsible for producing glucocorticoids, such as cortisol, which play a vital role in stress response, metabolism, and immune function. Supplements labeled as "adrenal cortex" typically contain extracts derived from animal adrenal glands. The traditional rationale for using adrenal cortex extracts is to support or "replenish" adrenal function, particularly in individuals experiencing fatigue or symptoms suggestive of adrenal insufficiency (sometimes termed "adrenal fatigue," though this is not a recognized medical diagnosis).
From a scientific standpoint, there is limited evidence supporting the use of adrenal cortex extracts to boost cortisol levels or support adrenal health in otherwise healthy individuals or those with non-classical adrenal issues. Historically, before the development of synthetic corticosteroids, adrenal cortex extracts were used to treat Addison's disease and other forms of adrenal insufficiency. However, these crude extracts have largely been replaced by standardized pharmaceutical preparations due to safety, efficacy, and dosing concerns.
Clinical studies on adrenal cortex supplements for general wellness or "adrenal fatigue" are lacking, and major medical organizations do not endorse their use for these indications. While there is a historical basis for their use in true adrenal insufficiency, current scientific validation for their use in supporting the "cortisol body system" in the general population is weak, warranting a rating of 2 out of 5.
