Vitamin D has been investigated as a supportive therapy in patients with congestive heart failure (CHF), primarily due to its known roles in cardiovascular health, modulation of inflammation, and regulation of calcium and phosphate metabolism. Several observational studies have noted that vitamin D deficiency is common among CHF patients and is associated with worse outcomes, including increased mortality and morbidity. These findings have prompted clinical trials to assess whether supplementation improves cardiac function or patient outcomes.

Results from randomized controlled trials have been mixed. Some small studies have shown modest improvements in left ventricular function and inflammatory markers with vitamin D supplementation, while others have failed to demonstrate significant benefits in terms of mortality, hospitalization rates, or quality of life. For example, the VINDICATE study (2016) found that high-dose vitamin D supplementation improved left ventricular function in patients with CHF, but larger trials have not confirmed significant clinical benefits. Recent meta-analyses generally conclude that while vitamin D deficiency should be corrected, routine supplementation in all CHF patients is not currently supported by strong evidence, and the improvements seen are relatively modest.
