Ribose is a naturally occurring sugar that plays a key role in the synthesis of adenosine triphosphate (ATP), the primary energy carrier in cells. Its use in supporting or treating Congestive Heart Failure (CHF) is based on the hypothesis that supplementing ribose may help replenish ATP stores in energy-depleted heart muscle, thereby improving cardiac function. Several small clinical studies and pilot trials have examined this possibility. For example, some research suggests that ribose supplementation can improve diastolic function and exercise tolerance in patients with CHF or ischemic heart disease. However, the sample sizes in these studies are often small, methodologies variable, and many are not double-blinded or placebo-controlled.

Reviews of the evidence, such as those from systematic reviews and the American Heart Association, generally conclude that while ribose shows theoretical promise, the current clinical evidence is limited and not robust enough to support widespread clinical use in CHF. There is no large, high-quality, randomized controlled trial to definitively confirm benefit. As a result, the evidence supporting ribose for CHF is rated low to moderate (2/5), and its use remains experimental. Patients considering ribose should consult with their healthcare provider, as it is not standard therapy for CHF.
