Marine oil, typically referring to fish oil rich in omega-3 fatty acids (eicosapentaenoic acid [EPA] and docosahexaenoic acid [DHA]), has been investigated for its potential benefits in managing congestive heart failure (CHF). Several randomized controlled trials and meta-analyses have explored the role of omega-3 supplementation in heart failure patients. The most notable is the GISSI-HF trial (2008), which enrolled over 6,000 patients with chronic heart failure and found a modest but statistically significant reduction in mortality and hospital admissions among those receiving 1g/day of omega-3 fatty acids compared with placebo. The proposed mechanisms include anti-inflammatory effects, antiarrhythmic properties, and improvements in endothelial function.
Guidelines from the American Heart Association and the European Society of Cardiology acknowledge that omega-3 supplementation may have a role in heart failure management, particularly in patients who remain symptomatic despite optimal medical therapy.
