Marine oil, typically referring to fish oil rich in omega-3 fatty acids (eicosapentaenoic acid [EPA] and docosahexaenoic acid [DHA]), has been investigated for its potential benefits in managing congestive heart failure (CHF). Several randomized controlled trials and meta-analyses have explored the role of omega-3 supplementation in heart failure patients. The most notable is the GISSI-HF trial (2008), which enrolled over 6,000 patients with chronic heart failure and found a modest but statistically significant reduction in mortality and hospital admissions among those receiving 1g/day of omega-3 fatty acids compared with placebo. The proposed mechanisms include anti-inflammatory effects, antiarrhythmic properties, and improvements in endothelial function.

Guidelines from the American Heart Association and the European Society of Cardiology acknowledge that omega-3 supplementation may have a role in heart failure management, particularly in patients who remain symptomatic despite optimal medical therapy. However, the magnitude of benefit is relatively small, and more recent studies have questioned the clinical significance of these effects.
