Plantain—typically referring to Plantago species such as Plantago major or Plantago lanceolata—has a long history of traditional use in herbal medicine for respiratory ailments, including sinus congestion. Traditional European, Native American, and Asian medicine systems have used plantain leaves as remedies for coughs, colds, and congestion, often preparing them as teas, syrups, or topical compresses. The reputed benefits are attributed to the plant’s mucilage content, which is believed to soothe irritated mucous membranes, and to its mild anti-inflammatory and astringent properties.

Despite this historical use, scientific validation is limited. Some in vitro and animal studies suggest plantain extracts possess anti-inflammatory, antimicrobial, and immunomodulatory effects, but there is insufficient clinical evidence specifically linking plantain to relief of sinus congestion in humans.
