Cinnamic aldehyde is a primary component of cinnamon oil and has been used traditionally in various cultures for respiratory ailments, including sinus congestion. The traditional use is likely based on the aromatic and mildly irritant properties of cinnamic aldehyde, which can stimulate mucous membranes and potentially promote a sensation of clearer airways when inhaled. In folk medicine, the inhalation of steam with added cinnamon oil is sometimes practiced for relief of nasal and sinus congestion.

However, there is limited scientific validation for the use of cinnamic aldehyde specifically in treating or supporting sinus congestion. Most available studies focus on its antimicrobial and anti-inflammatory properties, which are demonstrated in vitro and could theoretically be beneficial in the context of respiratory infections. Yet, clinical studies directly investigating its efficacy for sinus congestion in humans are lacking.
