Vitamin D is sometimes used to support or treat confusion, particularly in older adults, based on scientific observations linking vitamin D deficiency to cognitive impairment and delirium. Several observational studies have found that low serum vitamin D levels are associated with increased risk of cognitive decline and acute confusion (delirium), especially in elderly populations. The proposed mechanisms include vitamin D’s role in neuroprotection, regulation of neurotrophic factors, reduction of inflammation, and modulation of neurotransmitter synthesis.

However, the evidence supporting vitamin D supplementation as an effective treatment for confusion or delirium is limited and mixed. While some studies and reviews suggest that correcting severe vitamin D deficiency may help improve cognitive function or reduce the risk of delirium, randomized controlled trials have not consistently demonstrated a clear benefit of vitamin D supplementation for acute confusion.
