Aloe vera has a long history of traditional use as a remedy for various digestive complaints, including "atonic colon"—a term used to describe a sluggish or underactive colon often associated with constipation. In traditional herbal medicine, particularly within Ayurvedic and folk systems, the latex of the aloe vera leaf (the yellowish sap found just beneath the leaf skin) is known for its potent laxative properties. This effect is mainly attributed to compounds called anthraquinones, such as aloin and emodin, which stimulate bowel motility and increase water content in the intestines, thus promoting bowel movements. Aloe vera gel (the clear inner pulp), on the other hand, is not considered a laxative but is sometimes used for soothing gastrointestinal irritation.

Scientific validation for the use of aloe vera latex as a laxative exists, but evidence is limited and concerns about safety (especially with prolonged use) have restricted its recommendation in modern medicine. The efficacy of aloe latex as a stimulant laxative has been demonstrated in some clinical studies, but these are generally small and of low quality. Additionally, the use of aloe latex is associated with potential side effects such as abdominal cramps, diarrhea, and, with chronic use, possible kidney damage and electrolyte imbalance. Regulatory agencies such as the FDA have removed aloe-containing stimulant laxatives from over-the-counter sale due to safety concerns. In summary, while aloe vera has a traditional basis for use in atonic colon, modern use is cautioned due to limited scientific evidence and potential risks.
