Specialized pro-resolving mediators (SPMs) are a class of bioactive lipid mediators derived from omega-3 and omega-6 fatty acids, including resolvins, protectins, and maresins. Their primary role in the body is to actively resolve inflammation and promote tissue healing. In the context of colitis—an inflammatory condition affecting the colon—there is growing preclinical evidence suggesting that SPMs may be beneficial. Animal studies, particularly in murine models of colitis (such as dextran sulfate sodium-induced colitis in mice), have shown that administration of SPMs can reduce inflammatory markers, decrease tissue damage, and promote mucosal healing. These effects are attributed to SPMs’ abilities to limit neutrophil infiltration, enhance macrophage-mediated clearance of cellular debris, and stimulate anti-inflammatory cytokine production.
While these findings are promising, direct clinical evidence in humans with colitis (including ulcerative colitis or Crohn’s disease) remains limited. A few early-phase human studies and case reports suggest potential benefit, but robust, large-scale clinical trials are lacking as of 2024. SPMs are not yet part of standard medical therapy for colitis, but their mechanism of action is well-characterized, and their therapeutic potential is under active investigation. Thus, while the scientific rationale is strong and preclinical data are supportive, the current level of clinical evidence justifies a moderate rating.
