Proanthocyanidins are a class of polyphenolic compounds found in a variety of plants, particularly in fruits like grapes, cranberries, and apples. Their use for colitis, an inflammatory condition of the colon, is primarily based on preclinical (animal and in vitro) studies rather than robust clinical trials in humans. Research has demonstrated that proanthocyanidins possess strong antioxidant and anti-inflammatory properties. In animal models of colitis, such as those induced by dextran sulfate sodium (DSS), administration of proanthocyanidins has been shown to reduce colonic inflammation, decrease oxidative stress, and improve histopathological outcomes. The proposed mechanisms include inhibition of pro-inflammatory cytokines (such as TNF-α, IL-1β, and IL-6), suppression of NF-κB signaling pathways, and enhancement of antioxidant defenses.

However, there is a lack of substantial human clinical data to directly support the use of proanthocyanidins for treating or managing colitis. Most of the evidence comes from laboratory and animal studies, which, while promising, do not always translate to similar effectiveness in humans. Thus, while there is a scientific rationale and some experimental evidence for their potential benefit in colitis, the current level of evidence is limited and does not warrant strong clinical recommendations. More rigorous human trials are needed to establish efficacy and safety for this specific indication.
