Lactobacillus reuteri is a probiotic bacterium that has been studied for its potential benefits in managing colitis, particularly inflammatory bowel diseases (IBD) such as ulcerative colitis. The scientific evidence supporting its use is moderate. Preclinical studies in animal models have demonstrated that L. reuteri can reduce inflammation, enhance intestinal barrier function, and modulate the gut immune response. These effects are thought to be mediated through the production of anti-inflammatory compounds (such as reuterin and short-chain fatty acids), suppression of pro-inflammatory cytokines, and promotion of regulatory T-cells.

Several small-scale clinical trials in humans have investigated the impact of L. reuteri supplementation in patients with colitis. Some studies have reported improvements in symptoms, reductions in disease activity, and favorable changes in gut microbiota composition. However, the evidence is not unanimous: while some randomized controlled trials (RCTs) show benefits, others find little or no clinical effect compared to placebo. The variability in results may be due to differences in probiotic strains, dosages, patient populations, and study designs.

Overall, L. reuteri is considered a promising adjunct therapy for colitis, but more large, well-designed RCTs are needed to confirm its efficacy and to determine optimal strains and dosing regimens. Current guidelines do not universally recommend it as a first-line treatment, but it may be considered for some patients under medical supervision.
