Synopsis

Source of validity: Scientific
Rating (out of 5): 2

Lactobacillus delbrueckii, particularly as part of yogurt cultures or probiotic formulations, has been investigated for its potential benefits in managing colitis, especially ulcerative colitis and experimental models of inflammatory bowel disease (IBD). Some studies have shown that L. delbrueckii, often in combination with other probiotic strains (such as in VSL#3), may help reduce inflammation, modulate immune responses, and improve gut barrier function. For example, animal studies have demonstrated that L. delbrueckii can reduce colonic inflammation and mucosal damage in chemically induced colitis models. Proposed mechanisms include the inhibition of pro-inflammatory cytokines, enhancement of anti-inflammatory cytokines, and restoration of beneficial gut microbiota balance.

However, the direct evidence for L. delbrueckii alone (not as part of multi-strain preparations) in human colitis is limited, and most clinical trials have used mixed-strain probiotics. Systematic reviews and meta-analyses of probiotics for ulcerative colitis generally report modest benefits, but the specific contribution of L. delbrueckii remains uncertain due to this lack of targeted research. Therefore, while there is a scientific rationale and some supporting preclinical evidence, the overall quality and quantity of direct clinical evidence are limited, resulting in a moderate-to-low evidence rating.
