Lactobacillus acidophilus is a probiotic bacterium that has been studied for its potential benefits in managing colitis, particularly ulcerative colitis and other forms of inflammatory bowel disease (IBD). The rationale behind its use is based on the observed dysbiosis (microbial imbalance) in the gut microbiota of individuals with colitis. L. acidophilus is thought to help restore a healthier microbial balance, enhance intestinal barrier function, and modulate immune responses, thereby potentially reducing inflammation.

Scientific evidence for the use of L. acidophilus in colitis is modest. Several small-scale human clinical trials and animal studies have indicated some benefit, such as reduced inflammation and improved symptoms when L. acidophilus is used alone or as part of a multi-strain probiotic formulation. For example, studies have shown that L. acidophilus may decrease pro-inflammatory cytokines and increase anti-inflammatory responses in the gut. Systematic reviews and meta-analyses suggest that while probiotics as a group show some promise in maintaining remission in ulcerative colitis, the effects of L. acidophilus alone are less clear and often not statistically significant. Most guidelines recommend probiotics as an adjunct rather than a primary therapy, and more robust, large-scale trials are needed to clarify its effectiveness.
