Isomalto-oligosaccharide (IMO) is a prebiotic fiber that has been explored for its potential to support gut health. The scientific rationale for its use in colitis—specifically, inflammatory bowel diseases such as ulcerative colitis or Crohn’s disease—centers on its ability to modulate the gut microbiota and promote the growth of beneficial bacteria, such as Bifidobacteria and Lactobacilli. This, in turn, may help restore the balance of intestinal flora, which is often disrupted in colitis.

Several animal studies have suggested that IMO supplementation can reduce inflammation and improve colonic health. For example, research in rodent models of colitis has shown that IMO administration can decrease pro-inflammatory cytokines, enhance mucosal barrier function, and increase short-chain fatty acid (SCFA) production, particularly butyrate, which is known for its anti-inflammatory effects on the colon (Wang et al., 2019; Li et al., 2018).

However, clinical evidence in humans is limited. While some small studies and extrapolation from other prebiotics suggest possible benefit, there is insufficient data to strongly recommend IMO as a standalone treatment for colitis. Most evidence supports its use as an adjunct to other therapies, and more well-designed human trials are needed. Thus, while there is a scientific rationale and some preclinical support, the overall evidence remains moderate to low.
