Dwarf lilyturf (Ophiopogon japonicus) is a traditional Chinese medicinal herb, historically used for its anti-inflammatory and restorative properties. Recent scientific studies have started to explore its effects on colitis, particularly in preclinical models. Several animal studies have shown that extracts of Ophiopogon japonicus can reduce inflammation and improve mucosal healing in chemically-induced colitis in mice or rats. The mechanisms are thought to involve suppression of pro-inflammatory cytokines, inhibition of the NF-κB signaling pathway, and antioxidant effects. For example, saponins and polysaccharides isolated from the plant have demonstrated the ability to decrease levels of TNF-α and IL-6, which are key mediators in colitis pathogenesis. However, these findings are largely limited to laboratory and animal studies, with minimal clinical research in humans. There is no robust randomized controlled trial evidence supporting its efficacy in human colitis patients. Thus, while there is a scientific rationale and preliminary preclinical evidence, the overall strength of evidence is moderate to low, and its use cannot be fully endorsed for clinical practice at this time.
