Bifidobacterium (various species) has been the subject of multiple clinical and preclinical studies investigating its potential role in the management of colitis, particularly ulcerative colitis and, to a lesser extent, Crohn’s disease. The rationale for its use lies in its ability to modulate the gut microbiota, enhance mucosal barrier function, and reduce inflammation. Several randomized controlled trials (RCTs) and meta-analyses have explored the efficacy of specific Bifidobacterium strains, often in combination with other probiotics (as in the formulation VSL#3), in reducing disease activity and maintaining remission in ulcerative colitis patients. Some studies report modest benefits, such as improved clinical symptoms and reduced inflammatory markers, while others show no significant effect. The evidence is strongest for mild to moderate ulcerative colitis, with less robust data for other forms of colitis, such as microscopic colitis or Crohn's disease. The mechanism of action is thought to involve modulation of immune responses, competitive inhibition of pathogenic bacteria, and enhancement of mucosal healing. Despite this, there is heterogeneity in study designs, patient populations, and probiotic formulations, which complicates interpretation. Major gastroenterology guidelines recognize probiotics, including Bifidobacterium-containing formulations, as a possible adjunct for ulcerative colitis but do not recommend them as first-line therapy. In summary, while there is scientific evidence supporting the use of Bifidobacterium for colitis, particularly ulcerative colitis, the overall quality and consistency of the evidence is moderate, justifying a rating of 3 out of 5.
