Bacillus species, such as Bacillus coagulans and Bacillus subtilis, have been evaluated as probiotics for gastrointestinal conditions, including colitis. Preclinical studies (primarily in animal models) suggest that certain Bacillus strains may reduce colonic inflammation and restore gut microbiota balance. For example, Bacillus coagulans has demonstrated anti-inflammatory effects and improved clinical indicators in animal models of colitis, such as decreased cytokine production and improved histological appearance of the colon.
Small human clinical studies have also been conducted. For instance, a randomized controlled trial in patients with irritable bowel syndrome (IBS) and diarrhea-predominant symptoms—a group that sometimes overlaps with mild colitis—showed modest benefits in symptom relief with Bacillus coagulans supplementation. However, direct evidence in patients with ulcerative colitis or Crohn's disease is limited. The European Society for Paediatric Gastroenterology, Hepatology and Nutrition (ESPGHAN) and other expert groups generally do not recommend Bacillus probiotics as standard therapy for inflammatory bowel disease due to insufficient robust evidence.
In summary, while there is some preclinical and limited clinical evidence supporting the use of Bacillus species in colitis, the overall quality and quantity of data are low, and routine clinical use is not widely endorsed. More rigorous, large-scale human studies are needed to establish efficacy and safety for this indication.
