Colocynth (Citrullus colocynthis), also known as bitter apple, has a long history of use in traditional medicine, particularly in systems such as Unani and Ayurveda. Traditionally, colocynth has been used as a remedy for a range of gastrointestinal issues, including colic in children. The use is primarily based on the plant’s strong purgative and antispasmodic properties as described in historical medical texts. However, the evidence supporting its efficacy and safety for treating colic in children is extremely limited to anecdotal and traditional reports. There are no well-designed clinical trials or robust scientific studies validating its use for this indication. Moreover, colocynth contains compounds (notably cucurbitacins) that are known to be highly toxic, and its use—especially in children—is associated with significant risks, including severe gastrointestinal irritation and poisoning. Modern medical practice does not recommend colocynth for pediatric colic due to these safety concerns and lack of scientific validation. Therefore, while colocynth is cited in traditional literature for colic, there is minimal substantiated evidence, and significant safety risks preclude its use in contemporary pediatric care.
