Spearmint (Mentha spicata) has a long history of traditional use in herbal medicine, particularly for soothing digestive complaints and as a mild remedy for respiratory discomforts, including symptoms associated with colds. In folk medicine, spearmint preparations such as teas and inhalations have been used to help clear nasal passages and alleviate mild congestion, likely due to their menthol content, which imparts a cooling sensation and may provide subjective relief. However, compared to peppermint (Mentha piperita), spearmint contains much lower concentrations of menthol, the compound most associated with decongestant effects.

Scientific validation for spearmint as a decongestant is limited. Most studies on menthol’s decongestant effect focus on higher-menthol plants like peppermint or on synthetic menthol itself. There is little direct clinical evidence supporting spearmint's efficacy as a decongestant or its use for treating cold symptoms. Thus, while its use for this purpose is rooted in tradition, the scientific evidence supporting spearmint specifically for cold-related congestion is weak or lacking. Overall, its role in cold care is primarily historical, and it should not be regarded as a proven or potent decongestant based on current scientific standards.
