Synopsis

Source of validity: Scientific
Rating (out of 5): 3

Sambucus nigra, commonly known as elderberry, has a longstanding history of traditional use for treating colds and flu-like symptoms, particularly in Europe. However, scientific research in recent decades has begun to investigate its efficacy more rigorously. Several randomized, placebo-controlled clinical trials have examined elderberry extract's effects on upper respiratory symptoms. Notably, a 2004 study published in the journal Journal of International Medical Research found that elderberry extract reduced the duration and severity of influenza symptoms compared to placebo. Another 2016 meta-analysis suggested elderberry supplementation might reduce the duration and severity of cold symptoms in air travelers.

The proposed mechanism is that elderberry's bioactive compounds, especially flavonoids like anthocyanins, may block viral entry into cells and modulate inflammatory cytokines, which could account for symptomatic relief. However, study sample sizes have been relatively small, and not all trials are of high methodological quality. While results are promising, further large-scale, high-quality studies are needed to confirm efficacy and clarify optimal dosing and formulation. In summary, the use of Sambucus nigra as a decongestant and for cold symptom relief is supported by both tradition and some scientific evidence, though the strength of evidence is moderate rather than definitive.
