Gelcap is a dosage form, not a specific medication; it refers to a gelatin capsule that contains medication in either liquid or semi-solid form. For treatment of colds (especially as a decongestant), many over-the-counter (OTC) products use the gelcap format to deliver active ingredients such as pseudoephedrine or phenylephrine. Both of these are decongestants with a long history of use in cold medications.

Pseudoephedrine has robust scientific evidence supporting its efficacy as a nasal decongestant. Multiple randomized controlled trials and meta-analyses have demonstrated its ability to reduce nasal congestion in the context of upper respiratory tract infections (i.e., colds). However, due to regulatory restrictions related to its potential use in the illicit manufacture of methamphetamine, its availability has been limited in some countries, and many products now use phenylephrine instead.

The evidence for phenylephrine as an oral decongestant is considerably weaker. Recent large-scale analyses and systematic reviews have found little to no clinically meaningful benefit over placebo for nasal decongestion when taken orally, though it may still be somewhat effective when administered intranasally.

The use of gelcap formulations is primarily for convenience and rapid release, not because the capsule itself treats colds. The scientific evidence rating is 4, reflecting strong support for pseudoephedrine’s efficacy, but less so for phenylephrine. The use of these decongestants in gelcap form is scientifically justified for cold-related nasal congestion, depending on the active ingredient.
