Borneol is primarily used in Traditional Chinese Medicine (TCM) and other traditional healing systems as a remedy for colds, particularly for its decongestant effects. In these traditions, borneol is believed to help "open the orifices," clear phlegm, and facilitate the flow of qi, which is thought to support respiratory health and alleviate symptoms such as nasal congestion. Borneol is often included in herbal formulas, inhalants, and balms for its purported ability to clear blocked nasal passages and relieve headaches associated with colds.

Scientific evidence supporting borneol's use for colds or as a decongestant is limited. Some in vitro and animal studies have shown that borneol possesses anti-inflammatory and mild antimicrobial properties, which could theoretically assist in managing upper respiratory symptoms. However, these studies do not directly demonstrate efficacy for cold symptom relief in humans, nor do they confirm a decongestant effect comparable to standard pharmaceutical agents. Systematic clinical trials in humans are lacking. Thus, while borneol’s use for colds is well-established in traditional practices, the scientific validation for this application remains weak and largely anecdotal.
