Red Sage (Salvia miltiorrhiza) has a long history of use in Traditional Chinese Medicine (TCM), primarily for cardiovascular conditions, blood circulation, and inflammation. Its use in supporting or treating cold sores (herpes simplex labialis) is based mainly on traditional practices rather than strong scientific evidence. In TCM, Red Sage is sometimes included in herbal formulations intended to "clear heat" and "detoxify," which may be interpreted as reducing viral activity or inflammation associated with cold sores. However, direct references to Red Sage as a standalone treatment for cold sores are limited and are largely based on its general anti-inflammatory and purported antiviral properties observed in laboratory settings.

Scientific validation for Red Sage in the specific context of cold sores is currently lacking. Few, if any, clinical studies have examined its efficacy against herpes simplex virus (HSV-1), the causative agent of cold sores.
