Glycyrrhizin, a compound derived from licorice root (Glycyrrhiza glabra), has been studied for its antiviral properties, including activity against herpes simplex virus (HSV), the causative agent of cold sores. Several in vitro studies have demonstrated that glycyrrhizin can inhibit HSV replication by interfering with the virus’s ability to penetrate and replicate within host cells. Some research also suggests that glycyrrhizin may reduce inflammation and promote healing of herpes lesions. However, the majority of supporting evidence comes from laboratory studies and animal models, rather than large-scale, high-quality clinical trials in humans.

A limited number of small clinical studies and case reports have explored topical formulations of glycyrrhizin for herpes labialis (cold sores), with some reporting a reduction in lesion duration and symptom severity. Despite these findings, the overall quality and quantity of clinical evidence is relatively low, and there is insufficient data to conclusively recommend glycyrrhizin as a first-line treatment for cold sores. Major clinical guidelines do not include glycyrrhizin in their recommendations for herpes simplex management. While its use is supported by promising preclinical research, more robust human studies are needed to validate its efficacy and safety for this indication.
