Citral, a key component in the essential oils of lemongrass (Cymbopogon citratus) and other citrus-scented plants, has demonstrated some antiviral activity in laboratory studies. Its use in products marketed for cold sores (herpes labialis, primarily caused by herpes simplex virus type 1) is based on limited scientific evidence. In vitro studies have shown that citral can inhibit the replication of herpes simplex viruses, likely by disrupting the viral envelope or interfering with viral entry into cells. However, most of these results are from cell culture experiments rather than clinical trials in humans.
A small number of clinical studies have evaluated topical preparations containing lemon balm (Melissa officinalis), which is rich in citral and other related compounds. Some findings suggest reduced healing time and symptom severity for cold sores, but these effects cannot be attributed solely to citral, as other phytochemicals are present. There are no large-scale, high-quality randomized controlled trials directly assessing isolated citral for cold sore treatment. Thus, while the laboratory data is promising and supports potential antiviral effects, the clinical evidence remains preliminary, and citral’s efficacy as a standalone agent for cold sores has yet to be firmly established.
