Picroside, primarily derived from the plant Picrorhiza kurroa, has a long history of use in traditional Ayurvedic and Unani medicine for liver disorders, including cirrhosis of the liver. Traditional texts and ethnobotanical surveys describe the use of Picrorhiza kurroa as a “hepatoprotective” herb, believed to detoxify and protect the liver from various insults. The main active constituents, picrosides I and II, are thought to possess anti-inflammatory and antioxidant properties, which are cited in traditional literature as beneficial for liver health. However, while preclinical studies (in vitro and animal models) suggest some hepatoprotective effects of picrosides—such as reduced fibrosis and improved liver enzyme profiles—there is a lack of robust clinical evidence in humans specifically for cirrhosis. No large-scale, high-quality randomized controlled trials have demonstrated the efficacy of picrosides for treating cirrhosis. Thus, its use in this context is mainly justified by traditional practice and preliminary experimental findings rather than definitive scientific validation.
