Terpene lactones are a class of compounds found primarily in Ginkgo biloba extracts, including ginkgolides and bilobalide. These compounds have been widely studied for their effects on cerebral circulation and cognitive function. Scientific research suggests that terpene lactones may exert beneficial effects on cerebral blood flow by antagonizing platelet activating factor (PAF), reducing blood viscosity, and promoting vasodilation. Multiple randomized, placebo-controlled clinical trials, as well as meta-analyses, have provided moderate evidence that standardized Ginkgo biloba extracts (which contain defined amounts of terpene lactones) can improve symptoms of cognitive decline and enhance circulation to the brain, particularly in elderly patients with mild to moderate dementia or cerebrovascular insufficiency.

The most robust evidence comes from studies using standardized extracts like EGb 761, which contains approximately 6% terpene lactones. In these studies, improvements in measures such as cerebral blood flow, cognitive performance, and activities of daily living have been documented. However, it is important to note that while the overall extract has demonstrated efficacy, the specific effects of isolated terpene lactones (as opposed to the full phytocomplex) are less well-documented. Nonetheless, the scientific consensus supports the use of Ginkgo-derived terpene lactones for supporting cerebral circulation, with an evidence strength of 4/5 due to the volume and quality of clinical data, though not all studies are uniformly positive.
