Picamilon is a synthetic compound formed by combining niacin (vitamin B3) and gamma-aminobutyric acid (GABA). It was developed in Russia in the 1960s and has been studied primarily in Russian clinical and preclinical research. The rationale for its use in supporting cerebral circulation stems from the vasodilatory effects of niacin and the purported neuroprotective actions of GABA. Studies from Russia reported that Picamilon could cross the blood-brain barrier, where it is hydrolyzed to release GABA and niacin, potentially resulting in both increased cerebral blood flow and anxiolytic effects.

Some clinical trials and animal studies have shown that Picamilon can improve cerebral microcirculation, especially in models of ischemia or cerebrovascular insufficiency. For example, it has been used in Russia for treating patients with cerebrovascular disorders, including chronic cerebrovascular insufficiency and recovery after stroke. However, the majority of these studies are small, not placebo-controlled, and published in Russian-language journals, with limited replication in Western scientific literature.

Overall, there is a scientific basis for the idea that Picamilon may support cerebral circulation, but the evidence is limited in quality and scope.
