Lumbrokinase is an enzyme complex derived from earthworms, traditionally used in East Asian medicine for circulatory disorders. Modern scientific research has focused on its fibrinolytic (clot-dissolving) properties. A handful of small clinical studies and animal experiments suggest that lumbrokinase can enhance fibrinolysis, reduce blood viscosity, and potentially improve microcirculation, including in the cerebral vasculature. Some Chinese clinical trials have reported improved outcomes in patients with ischemic stroke or cerebral infarction when lumbrokinase is used alongside conventional therapies, such as antiplatelet agents. However, the methodological quality of many studies is limited, with small sample sizes, lack of blinding, and limited replication outside of China. Mechanistically, lumbrokinase appears to break down fibrin and may inhibit platelet aggregation, potentially reducing the risk of clot formation that can impair cerebral circulation. There is, however, no large-scale, high-quality randomized controlled trial in Western populations confirming its efficacy or safety specifically for brain circulation or stroke prevention. Regulatory agencies have not approved lumbrokinase for these uses, and more rigorous studies are needed to establish clear clinical benefit and safety. In summary, while there is preliminary scientific evidence suggesting a role for lumbrokinase in supporting circulation to the brain by promoting fibrinolysis, the evidence base is limited and not yet robust enough for strong clinical recommendations.
