Acetyl L-carnitine (ALCAR) has been investigated for its effects on cerebral circulation and cognitive function. The rationale for its use is rooted in its ability to cross the blood-brain barrier and its role in mitochondrial energy metabolism. Some studies suggest that ALCAR may enhance cerebral blood flow and support neuronal health by improving mitochondrial function, reducing oxidative stress, and modulating neurotransmitter activity. Clinical trials in elderly patients and those with mild cognitive impairment or early dementia have shown modest improvements in cognitive performance, which may be partially attributed to improved circulation and energy utilization in the brain. However, the direct evidence specifically linking ALCAR supplementation to significant improvements in cerebral blood flow is limited and mixed. Most studies focus on cognitive benefits rather than direct vascular effects, and the improvements observed are generally modest.
