Superoxide dismutase (SOD) is an antioxidant enzyme that catalyzes the dismutation of superoxide radicals into oxygen and hydrogen peroxide, thereby reducing oxidative stress. In the context of Chronic Obstructive Pulmonary Disease (COPD), oxidative stress is considered a significant pathogenic factor contributing to airway inflammation and lung tissue damage. Some scientific interest has therefore focused on whether enhancing SOD activity, either via supplementation or pharmacological means, could be beneficial in COPD management.
Preclinical studies have demonstrated that SOD administration can reduce markers of oxidative damage and inflammation in animal models of lung injury and COPD. However, clinical evidence in humans is limited and inconclusive. A handful of small clinical trials have explored the efficacy of SOD or SOD mimetics in COPD patients, generally showing modest improvements in oxidative stress biomarkers, but with minimal or no significant clinical benefit in terms of lung function or symptom relief. Furthermore, oral SOD supplementation is controversial due to questions about its bioavailability—SOD is a protein and may be degraded in the digestive tract.
Current guidelines for COPD management do not recommend SOD as a standard therapy, and large, high-quality randomized controlled trials are lacking. Thus, while there is a plausible mechanistic rationale and some early scientific exploration, the evidence base is not robust, and SOD should not be considered an established or validated treatment for COPD at this time.
