Berberis species, particularly those containing the active compound berberine (such as Berberis vulgaris and Berberis aristata), have been studied for their effects on lipid metabolism and cholesterol levels. The scientific evidence supporting their use in managing high cholesterol comes primarily from clinical studies and meta-analyses that focus on berberine, an isoquinoline alkaloid found in Berberis species. Several randomized controlled trials have shown that berberine supplementation can significantly reduce total cholesterol, LDL cholesterol, and triglyceride levels, while modestly increasing HDL cholesterol. The proposed mechanisms include upregulation of LDL receptor expression and modulation of AMP-activated protein kinase (AMPK) pathways, which contribute to improved lipid profiles.
Systematic reviews and meta-analyses published in reputable journals between 2012 and 2022 consistently report that berberine, at dosages typically ranging from 500 to 1500 mg per day, leads to reductions in total cholesterol by approximately 20-30 mg/dL, and similar improvements in LDL and triglyceride levels, compared to placebo. However, most studies have been of moderate duration (2-6 months) and sample sizes are often relatively small. Importantly, the evidence pertains primarily to purified berberine or standardized extracts, rather than whole Berberis plant preparations, so results may not fully translate to all Berberis products.
Overall, current scientific evidence moderately supports the use of berberine-containing Berberis extracts for cholesterol management, but larger and longer-term studies are needed to establish long-term efficacy and safety.
