D-alpha tocopherol, a natural form of vitamin E, has been investigated for its role in reducing the side effects of chemotherapy, particularly chemotherapy-induced peripheral neuropathy (CIPN) and some forms of organ toxicity. Several small randomized controlled trials and meta-analyses have explored vitamin E supplementation as a protective agent against nerve damage caused by chemotherapeutic agents such as cisplatin and paclitaxel. The proposed mechanism involves vitamin E’s antioxidant properties, which may counteract the oxidative stress and free radical formation seen with certain chemotherapies. However, the evidence remains mixed and is not robust. Some studies demonstrate reduced incidence or severity of neuropathy with vitamin E supplementation, while others show no significant benefit. Furthermore, there are concerns that antioxidant supplementation during chemotherapy might reduce the efficacy of the treatment by protecting not just healthy cells but also cancer cells from oxidative damage, though this remains controversial and is highly dependent on the specific chemotherapy regimen and cancer type. Current clinical guidelines do not universally recommend vitamin E supplementation for chemotherapy side effect management, largely due to the lack of large-scale, high-quality trials and the potential for harm in some contexts.
