Coenzyme Q10 (CoQ10) is used as a dietary supplement in some cancer patients undergoing chemotherapy, primarily with the intention of reducing treatment-related side effects such as fatigue, cardiotoxicity, and oxidative stress. The rationale for its use comes from CoQ10’s role as an essential component in mitochondrial electron transport and its antioxidant properties, which may help protect cells from the oxidative damage induced by certain chemotherapeutic agents, such as doxorubicin.

Scientific evidence supporting CoQ10’s efficacy in this context is limited and mixed. Some small clinical studies and animal models have suggested that CoQ10 supplementation may reduce chemotherapy-induced cardiotoxicity without compromising the anti-tumor effects of the drugs. For instance, a few trials have reported improved markers of cardiac function in patients receiving anthracyclines (like doxorubicin) alongside CoQ10. Additionally, CoQ10 may help alleviate general fatigue and improve quality of life, although this has not been consistently demonstrated.

However, large-scale, high-quality randomized controlled trials are lacking. Safety data suggest that CoQ10 is generally well-tolerated, but concerns remain about potential interactions with chemotherapy drugs. As a result, leading cancer organizations do not currently recommend routine CoQ10 supplementation during chemotherapy, but acknowledge its potential based on preliminary data. In summary, there is some scientific basis for CoQ10’s use to mitigate chemotherapy side effects, but the overall quality and quantity of evidence is low, meriting a modest evidence rating.
