Alpha-ketoglutarate (AKG) has been investigated for its potential to reduce certain side effects of chemotherapy, particularly mucositis (inflammation and ulceration of the digestive tract). Preclinical studies and some small clinical trials suggest that AKG supplementation may help protect the intestinal mucosa and promote recovery after chemotherapy or radiation therapy. The proposed mechanisms involve AKG's role as an intermediate in the Krebs cycle, supporting cellular energy metabolism, and as a nitrogen scavenger, which may help maintain gut barrier function and modulate inflammation. Several studies, including randomized controlled trials in pediatric patients undergoing hematopoietic stem cell transplantation, have shown that AKG can reduce the severity and duration of oral mucositis and diarrhea. However, these studies are limited in size and scope, and the evidence is not yet robust enough to warrant widespread clinical adoption. Systematic reviews acknowledge the positive findings but emphasize the need for larger, high-quality studies to confirm efficacy and safety.
