Vitamin B6 (pyridoxine) has a long history of use in alternative and complementary medicine for the management of Carpal Tunnel Syndrome (CTS), primarily based on anecdotal reports and small, older studies. The rationale for its use stems from the role of vitamin B6 in nerve function and the hypothesis that deficiency could exacerbate nerve compression symptoms. During the 1970s and 1980s, several small uncontrolled studies and case reports suggested that B6 supplementation could relieve CTS symptoms, especially in individuals with low plasma pyridoxal phosphate levels. This led to widespread traditional use, despite inconsistent findings in later research.

More recent, well-controlled studies and meta-analyses have not substantiated a strong benefit of vitamin B6 supplementation for CTS in the general population, especially among those without clear deficiency. The American Academy of Orthopaedic Surgeons and other major guidelines do not endorse vitamin B6 as a primary treatment due to insufficient and conflicting evidence. While B6 is essential for nerve health, routine supplementation has not been shown to significantly improve CTS outcomes when compared to placebo or standard interventions like splinting or surgery. Therefore, the use of B6 for CTS is largely traditional, with minimal scientific support (evidence rating: 1/5), and may be considered only in select cases where deficiency is suspected or confirmed.
