Vitamin E, particularly in the form of alpha tocopheryl succinate, has been investigated for its potential role in supporting or treating cardiovascular disease (CVD) due to its antioxidant properties. The rationale is that oxidative stress contributes to the pathogenesis of atherosclerosis and other cardiovascular conditions, and antioxidants like vitamin E might help mitigate this process. Early observational studies suggested an association between higher vitamin E intake and reduced risk of cardiovascular events. This led to substantial public and clinical interest in vitamin E supplementation for heart health.

However, large, well-designed randomized controlled trials (RCTs) such as the HOPE and HOPE-TOO studies, as well as meta-analyses, have generally failed to show a significant benefit of vitamin E supplementation (including alpha tocopheryl succinate) in reducing major cardiovascular outcomes such as myocardial infarction, stroke, or cardiovascular mortality. Some studies have even reported potential adverse outcomes with high-dose supplementation. As a result, clinical guidelines do not recommend vitamin E supplements for the prevention or treatment of cardiovascular disease in the general population.

In summary, while the use of vitamin E for cardiovascular disease has a scientific basis and has been rigorously studied, the evidence supporting its efficacy is weak (rated 2/5), and supplementation is not recommended for this purpose based on current scientific consensus.
