Vitamin E (alpha-tocopherol acetate) has been investigated for its potential role in supporting or treating cardiovascular disease (CVD) due to its antioxidant properties. Early observational studies in the 1990s indicated that higher vitamin E intake might be associated with reduced risk of coronary heart disease, leading to widespread interest and use of vitamin E supplements for cardiovascular health. The rationale was that vitamin E, as a lipid-soluble antioxidant, could inhibit the oxidation of low-density lipoprotein (LDL) cholesterol, a process believed to contribute to atherosclerosis.

However, large randomized controlled trials (RCTs) and meta-analyses have not demonstrated significant cardiovascular benefit from vitamin E supplementation. Major studies such as the Heart Outcomes Prevention Evaluation (HOPE) trial and the Vitamin E Atherosclerosis Prevention Study (VEAPS) found no significant reduction in cardiovascular events or mortality among participants taking vitamin E compared to placebo. Some studies have even suggested a possible increased risk of heart failure or all-cause mortality at high doses.

In summary, while the use of vitamin E for cardiovascular disease is based on scientific hypotheses and early observational evidence, robust clinical trial data do not support its efficacy for prevention or treatment. Current guidelines do not recommend vitamin E supplementation for cardiovascular disease prevention in the general population.
