Nicotinamide Adenine Dinucleotide (NAD+) is a crucial coenzyme involved in cellular metabolism, redox reactions, and energy production. Interest in its role in cardiovascular disease (CVD) stems mainly from preclinical and early clinical studies suggesting that NAD+ levels decline with aging and in chronic diseases, including CVD. Lower levels of NAD+ have been associated with impaired mitochondrial function, increased oxidative stress, and inflammation, all of which are implicated in the pathophysiology of cardiovascular diseases.
Supplementation strategies, often using NAD+ precursors such as nicotinamide riboside (NR) or nicotinamide mononucleotide (NMN), have shown promise in animal models. These studies report improvements in vascular function, reductions in arterial stiffness, and enhanced cardiac function. Some small human studies have found that oral supplementation with NAD+ precursors can increase NAD+ levels and improve certain biomarkers related to cardiovascular health, such as blood pressure and endothelial function. However, these studies are preliminary, often with small sample sizes and short durations.
Currently, there is insufficient large-scale clinical evidence to conclusively demonstrate that NAD+ supplementation provides meaningful benefits in preventing or treating cardiovascular disease in humans. The current scientific validation is limited to mechanistic insights and early-phase trials. Therefore, while the use of NAD+ for cardiovascular disease is supported by scientific rationale and emerging evidence, the overall strength of clinical evidence remains modest.
