Ligustrazin (also known as tetramethylpyrazine or TMP) is a bioactive compound extracted from the Chinese herb Ligusticum chuanxiong, widely used in Traditional Chinese Medicine (TCM). Its use for cardiovascular diseases, such as angina pectoris, coronary heart disease, and stroke, is primarily justified by centuries of traditional practice in China. Historical TCM texts describe the herb as promoting blood circulation and removing blood stasis, which are concepts aligned with cardiovascular support in TCM theory.

Some preclinical studies and small clinical trials suggest that Ligustrazin may exert vasodilatory, anti-platelet aggregation, and neuroprotective effects, potentially benefiting cardiovascular and cerebrovascular conditions. However, these findings are mostly from animal models or poorly controlled human studies. Systematic reviews of the literature (e.g., Cochrane reviews, PubMed studies) generally conclude that the clinical evidence is insufficient due to methodological limitations, small sample sizes, and lack of rigorous randomized controlled trials. Therefore, while there is some experimental and anecdotal support for cardiovascular benefits, robust scientific validation is lacking. Its continued use is largely driven by traditional practice rather than high-quality clinical evidence.
